The interviewer instructions were inadvertently cut off of the first version of the master questionnaire by a program we use to auto populate the instrument with the data from the CATI script. That has been fixed and the corrected MQ has been posted. Thank you for noticing this as the rotation of this question is an important part of the methodological integrity of this study.
